There is a clear relationship between the thickness of the epicardial fat layer and the degree of myocardial inflammation and fibrosis.34 The epicardial fat volume is significantly greater in HFpEF patients, and many of the comorbidities associated with HFpEF, such as obesity, diabetes, and chronic kidney disease, also show epicardial fat increases.33 In HFrEF, the relationship is less clear, as both high and very low epicardial fat values have been reported in these patients.34 In addition, excess epicardial fat has been associated with a poorer prognosis in HFpEF.3335 A significant increase in any cause deaths and HF hospitalizations36 has been reported in patients with preserved or mildly reduced EF values, regardless of the body mass index and other comorbidities
